Waiver of Liability Relating to Coronavirus/COVID-19

 

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious.

 

COVID Waiver

 

1 - One Three Studios cannot prevent you or your player from becoming exposed to, contracting, or spreading COVID-19 while utilizing One Three Studios services.

 

2 - It is not possible to prevent against any presence of the disease. Therefore, if you choose to utilize One Three Studios services in which One Three Studios attends practice, training sessions, or events  you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.

 

3 - ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19.

 

4 - I hereby choose to accept the risk of contracting COVID-19 for myself and/or my player in order to utilize One Three Studios services.

 

5 - These services are of such value to me and/or to my player, that I accept the risk of being exposed to, contracting, and/or spreading COVID-19 in order to utilize One Three Studios services in person if applicable: “rather than arranging for an alternative method of enjoying the same services virtually (e.g. videoconference)]. 

 

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against One Three Studios and its owners, officers, directors, managers, officials, trustees, agents, employees, or other representatives in connection with exposure, infection, and/or spread of COVID-19 related to utilizing One Three Studios services. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen. CHOICE OF LAW: I understand and agree that the law of the State of Florida will apply to this contract. I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE, AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE.

 

Parental Photo Consent Form for Children/Minors

 

We recognize the need to ensure the welfare and safety of all young people taking part in any activity or service associated with One Three Studios.

 

1 - When you use our services or attend an  event One Three Studios is apart of , you enter an area where photography, audio, and video recording may occur. By attending the event or using our services , you consent to interview(s), photography, audio recording, video recording and its/their release, publication, exhibition, or reproduction to be used for news, web casts, promotional purposes, telecasts, advertising, inclusion on websites, or any other purpose by One Three Studios.

 

2 - You release One Three Studios, its officers and employees, and each and all persons involved from any liability connected with the taking, recording, digitizing, or publication of interviews, photographs, computer images, video and/or or sound recordings.

 

3 - By attending the event, you waive all rights you may have to any claims for payment or royalties in connection with any exhibition, streaming, web casting, televising, or other publication of these materials, regardless of the purpose or sponsoring of such exhibiting, broadcasting, web casting, or other publication irrespective of whether a fee for admission or sponsorship is charged. 

 

I consent to having my child photographed or recorded for this event.
